Drug-resistant gonorrhea a concern in Ontario

A new study raises concerns about the spread of drug-resistant gonorrhea in Ontario.

A strain of gonorrhea that responds poorly to the last class of drugs available to treat the bacteria is gaining ground among the strains spreading in the province, data presented Tuesday at an infectious diseases conference in Chicago suggest.

The strain wasn't seen in the province in 2005, but by May of this year made up 11 per cent of a representative sample of gonorrhea isolates tested by the provincial laboratory, Dr. Vanessa Allen of Public Health Ontario reported at the Interscience Conference on Antimicrobial Agents and Chemotherapy.

The majority of cases that have been spotted were in men in Toronto. But the strain has been seen elsewhere in the province as well as in other parts of Canada and around the globe, said Allen, a medical microbiologist.

New approaches to treating and controlling the sexually transmitted disease need to be adopted before the bacteria outsmarts all the drugs available to treat the infection, she suggested.

"There's a very imminent threat that there will be nothing left to treat this infection with. And unless we're very careful now, I really think that we're at risk of getting there sooner rather than later."

Public Health Agency of Canada confirmed Tuesday its experts are working on revised guidelines for testing and treating gonorrhea.

"The more aggressive we can be, the more chance we have of eradicating this drug-resistant clone," Allen said in an interview from Chicago.

On Monday, the Canadian Medical Association Journal warned in an editorial that the threat of widespread multi-drug-resistant gonorrhea demands an urgent public response.

"Without action, we are heading back to the pre-antibiotic era, with an escalation in the number of deaths from other multi-resistant organisms as well as rampant gonococcal infections," said the editorial, signed by several editors including journal editor Dr. Paul Hebert.

Known as "the clap" in bygone days, gonorrhea can be challenging to control because some people who are infected don't have symptoms. Curing the infection does not provide lifetime protection, as is seen with some diseases; a person can be reinfected with gonorrhea.

The bacteria are spread by sexual contact and infection can occur in multiple sites in the body. But doctors often don't look for signs of infection in places like the throat or anus, Allen said, which contributes to the number of cases that are missed.

In women, untreated gonorrhea can cause pelvic inflammatory disease which can lead to chronic pelvic pain and infertility.

Babies born to women with gonorrhea may suffer from blindness or life-threatening blood infections. The bacteria can also cause infertility in men.

If the bacteria move to the blood or joints, infection can be fatal. Infection with gonorrhea increases one's risk of being infected with HIV; and HIV-positive people with gonorrhea can transmit HIV more easily to sexual partners.

Gonorrhea superbug resistant to all antibiotics

A superbug strain of gonorrhea that’s resistant to all available antibiotics has been discovered in Japan, posing a potential threat to public health worldwide, infectious disease experts say.

An international team of researchers identified the highly drug-resistant strain of Neisseria gonorrhoeae — the bacterium that causes the sexually transmitted infection — through genetic analysis.

Dubbed H041, the strain has extreme resistance to all cephalosporin-class antibiotics, the last remaining drugs still effective in treating gonorrhea.

“This is both an alarming and a predictable discovery,” researcher Dr. Magnus Unemo of the Swedish Reference Laboratory for Pathogenic Neisseria, said in a statement. “Since antibiotics became the standard treatment for gonorrhea in the 1940s, this bacterium has shown a remarkable capacity to develop resistance mechanisms to all drugs introduced to control it.

“While it is still too early to assess if this new strain has become widespread, the history of newly emergent resistance in the bacterium suggests that it may spread rapidly unless new drugs and effective treatment programs are developed,” said Unemo, who presented the team’s findings Monday at the International Society for Sexually Transmitted Disease Research meeting in Quebec City.

A spokesman for the Public Health Agency of Canada said the federal agency is not aware of any cases of H041 strain in this country.

Gonorrhea is one of the most common STIs in the world and can cause infertility, increase HIV transmission, and have profound effects on infants born to infected mothers, including blindness. Left untreated, the disease can be fatal.

Dr. Vanessa Allen, a medical microbiologist at the Ontario Agency for Health Protection and Promotion, said she’s not surprised by the emergence of a superbug gonorrhea strain because the infectious agent is known to exchange genetic material with other organisms in the genital tract, which helps confer resistance.

There’s been a long history of Neisseria gonorrhoeae losing its susceptibility to drugs used to treat it, including the penicillin class of antibiotics first introduced about 70 years ago.

“And, in fact, we’re now on the fifth or six class of antimicrobials,” Allen said Monday from Quebec City, where she was attending the conference.

Cephalosporins, which are administered in pill or a more potent intravenous form, are the latest to lose their ability to wipe out infection caused by the new strain.

With no alternative drugs in the pharmaceutical pipeline, “the concern is that we’ll go to potentially an era where it will be like the pre-antibiotic era,” she said. “So people again will see consequences of chronic gonorrhea infection that are associated with neonatal infections, infertility and those things.”

hey guys, here is a quick fact about one of our pet peeves from the Toronto Public Health...

Chlamydia

What is it?
Chlamydia is the most common reportable Sexually Transmitted Infection (STI). It is
caused by a bacterium called Chlamydia trachomatis.

How is it spread?
You can get chlamydia from unprotected oral, vaginal, or anal sex with an infected person. It can take 2 to 6 weeks or longer for symptoms to appear. Sometimes, you can get chlamydia in the eyes from contact with your hands or direct contact with infected fluids. An infected mother can also pass it to her baby during delivery. An infected person can pass it on at any time until he or she is properly treated. Untreated chlamydia can live in the body for months or years.

What are the symptoms?
More than 75% of women and 50% of men infected with chlamydia have no symptoms. They can pass the infection on to their sexual partner(s) without knowing. Women with symptoms may have increased vaginal discharge or irritation. Other symptoms may include pain during urination, bleeding during or after sexual intercourse, pain in the lower abdomen or pain during sexual intercourse. Men may have discharge and itching around the urethra (opening in the penis). They may also have burning when they urinate, or pain and swelling in the testicles. These symptoms may come and go.

How is it diagnosed?
A health professional can test women for chlamydia by taking a sample of fluid from the cervix. The cervix is the opening to the womb (uterus). In some cases, they use a urine test to check for chlamydia in women. For men, it’s usually a urine test. People should not urinate for 1 hour before this test. In some cases, they may take a sample of discharge from a man’s urethra. Sometimes they also test the throat or rectum for the infection.

What are the complications?
Untreated chlamydia in women may lead to a condition called pelvic inflammatory disease(PID). It is a serious infection of the lining of the uterus (womb), the fallopian tubes and the ovaries. Some women will have long-term pelvic pain. Sometimes, if a woman has PID, her fallopian tubes become blocked by scar tissue. In this case, sperm may not be able to fertilize an ovum (egg). She will not be able to get pregnant. If the fallopian tubes are only partly blocked so that sperm can pass through, she may develop a pregnancy inside a fallopian tube. This is called an ectopic(tubal) pregnancy, which can be very dangerous. A pregnant woman with untreated chlamydia may have a miscarriage. If she gives birth without being treated, her baby may become ill with pneumonia or eye infections. A man who has chlamydia and goes untreated may have swelling and pain in his testicles. This condition is called epididymitis. Lack of treatment may also cause the tubes that carry the sperm may get blocked by scar tissue, causing sterility.

How is it treated?
Chlamydia is treated with antibiotic pills. Some of the commonly used antibiotics include Azithromycin, Doxycycline, Erythromycin and Amoxicillin. It is important for a person diagnosed with chlamydia and their partner(s) to take all their medication as directed by their doctor or clinic. Even if the symptoms have disappeared, it is important to take all the pills to make sure that the infection is cured. Do not have sex while you and your partner(s) are on the medication and for seven days after you finish it, even with a condom.

What about the sexual partners?
It is important to inform all of your sexual partners within the past 60 days so that they can be tested. If a person with chlamydia has not had sex in the past 3 months, then their last sexual partner should be tested and treated. Any sexual partners need to be told that having no symptoms does not mean there is no infection. If you do not want to tell your partners, a public health nurse can notify them for you. They will not give your name.

Is follow-up important?
In some cases, the doctor will advise a follow-up test to make sure the infection is cured. This follow-up test should be done 4 weeks after you have finished taking all your pills.

Remember:
• Take all medication as prescribed by the doctor or clinic.
• Avoid sex for 7 days after you and your sexual partner(s) have completed
treatment. A person can get re-infected every time they have sex with an untreated
partner.
• If you have an untreated STI like chlamydia, it is easier to get HIV from a person
who has it.
• Use condoms to lower the chance of getting STIs.
• It is possible to have more than one infection at a time, so it is important to be
tested for other STIs.

In Toronto, we are seeing more cases of syphilis in men having sex with men (MSM). The same thing has happened in Vancouver and Ottawa. According to the AIDS Committee of Toronto, Toronto saw 31 cases in 2001, 195 cases in 2002, 326 cases in 2003 and 300 cases by mid-November 2004.

How do I know if I have it?

You may notice a painless sore on your penis or near your anus (bum). The sore will go away by itself. Many people do not notice the sore because it is painless. You probably wouldn’t notice one in your throat, but you could get it there, too, from unprotected oral sex.

This first sign of syphilis will show up about three weeks after you have sex with someone who has it. Four to 10 weeks later, you may notice a rash on your body, palms and soles of your feet. The rash will also go away on its own.

The late stage of syphilis usually happens 10 to 20 years later. It can occur much earlier if you also have HIV. Late stage syphilis can cause serious heart, brain and bone disease.

Can you cure it?

Yes. Syphilis can be easily treated and cured with antibiotics.

How can I get tested?

Your doctor can order a blood test for syphilis; or you can go to a sexual health clinic.

How can I protect myself?

As usual, the best protection for you and your partner(s) is using a latex condom every time you have oral, anal or vaginal sex. Consider getting tested for syphilis if you think you may be at risk

If someone you had sex with within the last three months tells you they have syphilis, get tested. Even if the test is negative, you should be treated with antibiotics. The test results will help the doctor make any other decisions about your treatment.

Then to see a whole array of men, sitting around in the lounge area, eating a turkey dinner practically naked. There is a Christmas tree, Christmas decorations, and even Christmas music. So it is sort of like a party.

Who are some of the men that go to the baths on Christmas Day? A few years ago, I did some internal surveying. I asked, "Why are you here on Christmas Day?" Some of the men had finished celebrating with their family very early in the day. With nothing to do in the afternoon, a bathhouse was chance to kill some time. Then there were the men who were not out to their families. By putting up a charade for the most of the day, some got anxious and uptight. Bathhouses need customers to be profitable. How would they be able to keep customers if they leak personal information? California cops recently snagged Angel Pena Ayala, a 24-year-old felon who used Craigslist to rob gay victims at gunpoint. And it reminded us that Craigslist's M4M section is not only a good place for "RAW BB BTTMS ISO GROUP LOADZ." 1. THE STABBING SATANIST

WTF HAPPENED?: When WABC Radio Reporter George Weber posted an ad looking for "violent sex", he met Satanic 16-year-old knife fetishist John Katehis. Weber wanted to pay Katehis $60 to smother him with a pillow. But instead, Katehis tied Weber's ankles with duct tape and stabbed Weber 50 times in the neck and body.

LESSONS LEARNED: If Weber had checked out Katehis' MySpace page before inviting him over, he might have seen his creepy-ass knife collection and friendly warning, "If you disrespect me then I will fucking break your neck."

And remember, statuatory rape laws exist for the good of both children and adults. Sixteen-year-olds make horrible lovers, under any circumstances.

2. THE CRAWFORD FAKE

WTF HAPPENED?: Apparently Gossip Girl star Chace Crawford likes to work out at the gayest gym in all of New York (Equinox in the Meatpacking District). But it certainly doesn't help his gay rumors much.

LESSONS LEARNED: Try not to work in a place staffed by douchebags. At least you know how to celebrate your frienemy's birthday this year.

5. THE CRAIGSLIST KILLER

WTF HAPPENED?: Another Albany med student (what's wrong with that place?) 22-year-old Philip Markoff used Craigslist to lure sex workers to their death. "The Craigslist Killer" shot a 26-year-old masseuse to death, robbed an exotic dancer, and held up a Las Vegas hooker—he even went after transexuals. Police eventually arrested him after linking him to three crime scenes via video footage.

As the fresh piece of meat in an otherwise "stale" relationship, you'll be regarded with the interest and fascination usually reserved for the re-appearance of the Dodo bird. In most cases, you may even find yourself treated like visiting royalty and having your every sexual whims and fancies attentively attended to.

Although sex with a couple entails more bodies, more heat and more excitement, a sexual liaison with a couple can also prove to be extremely hazardous to one's self-esteem, health and reputation.

So unless you're into double trouble or enjoy being on the receiving end of a tag-team effort to boot you out of the house (or worse, the gay community), you would do well to bear in mind Fridae's Five Commandments - before, when and after having sex with a gay couple.

Commandment No. 1: No Playing Favourites
It's inevitable. You're going to be more attracted to one of the guys than the other. But unfortunately, you can't play favourites. Still if you're the homo-equivalent of the Oscar winning Meryl Streep and can convincingly feign equal interest, then this would not pose a problem and you can skip to Commandment Number Two. But if you lack the formidable acting prowess of the divine Ms Streep, then it's best to keep both your eyes closed. That way, you won't know who you are deep-throating or whose mouth is having a go at your boner with the suction power of an industrial strength vacuum cleaner and better yet, you'll be way past caring.

Commandment No. 2: No Sleepover Unless

Making The Connection

It has dawned on me that there is an entirely different reason (other than the sex) as to why so many gay men go to the baths. What is it? It is the need for gay men to connect with other gay men.

Why is there this need to make a connection with someone else? One word – loneliness: Or rather a subconscious feeling of loneliness. Despite all of the strides and advances that have been made for gays and lesbians in the mainstream society, it can still feel very isolating being 'gay' in the gay community. There are many men that just don't feel they fit in.

And it is even worse if you have a lesser status in the gay community. Many gay men have self-esteem and insecurity issues. So it is very common to see gay men bash their fellow gay men who are viewed to be 'lower class'. Whether it is being a different race to driving a used car, there is nothing anyone can do to change the elitist gay environment that is out there. Just remember it says more about the person saying it, than it does about the group it is being said about. These people are the ones with a problem. So with all of the bars and clubs filled with all of these pretension and superficial gay men, where is there a safe haven to hang out with other gay men and feel somewhat accepted? You guessed it, the baths.

True, the baths can be as superficial as the bars and clubs. But it is also a place where no one is ostracized that badly. And everyone is on somewhat of an equal level. There is no class distinction because everyone is naked, in a towel. Lawyers interact with truck drivers and so on. Plus, the baths is the only gay environment where you can see every type of gay man regardless of age or race. The bathhouses are a behind closed doors atmosphere where anonymity reigns supreme. Most times that connection you are making with someone else is a sexual one. But underneath those sexual feelings, lies the need to connect. Look at how many glory holes there are in sex clubs. And how much anonymous blowjobs go on in bars, clubs, dance halls, and even in public bathrooms. Again, underneath all of this oral activity is the need to connect with someone else, no matter how short or fleeting the experience may be.

For instance, I've seen this one white guy at the baths for the past few years. I've spoken to him a few times, and he is totally gay, very friendly and cute too! I've seen him at all hours of the day; after work, before Sunday dinner, on a Saturday afternoon, and so on. And all he does is gives blowjobs any chance he gets. And this is probably what the deeply closeted guy, and others like him, goes through each and every time.

But there are also many gay men who go to the baths, looking for some sort of companionship in a non-sexual way. That is why you see more married, bi or closeted men at the baths than any other gay environment (i.e. bars or clubs). These men don't have many dealings interacting with the gay community and the need to connect someway with other gay men is strong. Just like I had mentioned with the deeply closeted guy, that need is actually insecurity and denial about their sexuality. By connecting with another gay man, these men are able to fill up that empty vacuum of insecurity. Thus they feel secure. But only for a short period of time, before that need resurfaces. What the baths offers these men is a false sense of security and acceptance. There is a sense of freedom to let go and feel uninhibited, outside of their normal lives. And since the baths is such an anonymous and closed doors environment, these men feel safer connecting with other gay men there. The bars or clubs, which is very visible, don't offer the same type of anonymity the baths can offer.

But it is not only these 'straight' men having this need to connect. Gay men have this need to. That is why you can see so many of the same men at the baths over and over again. All they do is hang out. It is almost like being a regular at the same bar night after night. Why are there these regulars? Like the deeply closeted guy, it is the subconscious feeling of loneliness and the need to connect with other gay men. To feel a sense of belonging within the gay community.

For instance, I've seen this good-looking Russian man frequent the baths on a regular basis. And how do I know he is Russian? Anyone who will listen, he has told his life story to. Complete in a heavy Russian accent. Any White guy the Russian spotted, he would approach and talk to about everything and anything. And some times, the Russian would hit it off with someone and get lucky. But just a warning; some guys do find this annoying when complete strangers come up and starts talking. Again it just gets in the way of some men looking for pure sex (which underneath is a need to connect). When approached for conversation, some men think that sex is the motivating factor

I've overheard various bits of conversations the Russian has had with other White guys. What I can piece together is that he is not OUT at all. But has these urges to be with other guys. He also considers himself to be straighter, than gay. Seeing this Russian talk to every White guy and try to be very social, struck me as a desperate need to connect with the gay community. I've even overheard the Russian guy admit to other people that he uses the baths to socialize and meet other gay men. Because the fact is he doesn't know where else to go to meet other gay men.

But another thing struck me about the Russian. After a while I couldn't help noticing that he spoke to almost every regular at the baths, old and young, fat and thin. Everyone that is, but myself and all the other visible minorities. When I, or other visible minorities, would be in the whirlpool with him, he never spoke to us. Not once. However, the minute anyone White would get into the whirlpool, he would immediately turn on the charm and start a conversation. You could almost see the desperation in his eyes, even in his body language, as he wanted to connect with someone, anyone. But as long as they were white

I know I'm speaking about racial thing again. But clearly it is just human nature for people to only feel safe and secure around other people like themselves. So a White Russian guy probably would only feels comfortable around other, similar white guys. Since he probably wasn't surrounded by to many visible minorities in Russia. He also probably feels a sense of 'foreignness' around anyone who isn't White. But he also seemed to be very class and status conscience, which I mentioned as being prevalent with gay men earlier. Hence only speaking and interacting with other whites guys.

Another group that is very class conscience is Asians. Yes, I'm on that subject again, but this next anecdote is too good not to tell. The other day I was at the baths, and I saw this VERY muscular twenty something Asian guy (and yes, the Russian guy ignored him to!) He seemed quite agitated, walking up and down the halls quite impatiently. Finally he was paged to go to the front desk to either check out or renew his room. But he wasn't ready to go. So he checked out of his room and purchased a locker, so he could stay longer. I had later heard from the front desk attendant that he had been at the baths for 8 hours at that point. He must have needed to get laid BAD! As time wore one he was getting more and more bitchy. At least that is what the bathhouse staff thought.

The next thing I knew, he checked out of his locker, re-purchased another room, and was in the company of a white Twinkie, who was a hustler. All of a sudden his personality changed. No longer was he rude and scowling at other people. But he seemed to be in a buoyant mood, laughing and jovial in the company of his white rent-boy. And the thing is, they hardly had any sex! They spent most of their time talking in their room, in the whirlpool, or sitting in the sauna. I'm sure they had sex at some point. But it was almost like the Asian guy wanted and needed some company, some conversation. And it had to be from someone who was white.

Just look at the money he spent on that one day. For all of his time at the baths (over 12 hours) and renting that white guy just for conversation, it cost him about $150.00.

When I observe the Russian guy or the Asian guy, I see myself about five years ago. The need to connect, to feel acceptance, to feel a sense of belonging in the gay community. Through therapy, I don't have those desperate feelings to connect with someone else anymore. I feel more secure and OK being alone. And you know what? It's a great feeling.
